Hmm, how come? I guess falling down with the dildo inserted could cause damage but as long as there’s always someone close by to help if needed, I would consider OBPs pretty safe.

Well say the person slips or gets unconcious. In that moment you basically have a metal pole that will inflict blubt trauma on this persons internal organs. To much blunt trauma on someones uterus is a lifethreatening situation. Thats why impaling was a medival death sentence.

I see your point but I still wouldn’t call OBPs any more dangerous among kinky activities and the risks you mention can and should mitigated with standard BDSM safety measures. Also, it’s not clear from this picture, but usually the top part which actually gets inserted into the sub is a dildo or something else with a fair bit of flex to it, of course it’s not just solid metal pike.

If the dildo is structurally sound -so mostlikly metal too - it will reduce the fall length otherwise it wont matter. You have to understand that it would be like you standing on a dildo that is inserted into someone with you full body weight and i cant state enough how much damage that would cause. I cant think of a kink with a higher risk potential beside wrongly done choking.

If you want to mitigate this slightly you would need to have a T bar that handels the weight

Again, the risks you mention are certainly real. However, I believe they can be almost entirely mitigated by always having someone present who’s ready to catch or assist the sub if they start to fall, or to release them immediately if needed. That’s pretty standard stuff in BDSM. Clear communication and closely monitoring the sub are the key. Also, it’s very unlikely that someone would suddenly collapse or lose consciousness just like that without any prior warning. I’m not saying it could never happen, but the risk is quite low.

If proper safety precautions aren’t followed, then yes, there’s a risk of serious injury when using devices like this. But that’s also true for many other higher risk scenes. I just don’t see OBPs as riskier than things like suspension bondage or CBT/ballbusting, for example. In my opinion, the risks involved are different, but not greater or lesser. We can agree to disagree on that.
