I’m wondering because it definitely does when they are transforme, but what about before the magic/curse takes affect? Are they just normal humans until it kicks in and sets off the medallion?

The medallions vibrate in response to magic in all its forms, including curses, charms, and spells.

From reading this wiki, I would say yes. It does warn of the curse afflicting a werewolf, even in human form.
See I didn’t know because in the first book Nivellen doesn’t set it off if I remember correctly, but his house does. And he is a cursed being.
Pretty sure it does vibrate near them
